Fishbone diagrams or Ishikawa Diagram is a commonly used tool to determine cause and effects, helping individuals and organizations achieve better root cause analysis. The Fishbone Diagram template is an excel based solution that includes two versions, one with easier, simpler visual while the second provides space for sub-causes.
To use the Fishbone Diagram v1, simply open the file, go to the relevant worksheet and begin filling out the cells for ‘Description’, ‘Cause’ and ‘Issue or Outcome’.
This version of the Ishikawa Diagram loses a bit of its ‘fish’ structure in exchange for the creation of sub-causes, an essential piece for many detail-oriented individuals. Similarly, to the Fishbone Diagram v1, all one has to do is to fill out the ‘Description’, ‘Cause’, ‘Issue or Outcome’ and include the ‘Sub-causes’.
To print either of the Fishbone Diagrams, go to the relevant worksheet, then press CTRL+P or go to File->Print.
